We would be very grateful if you would provide your patient with all relevant findings, as well as a referral or admission form stating the corresponding question. Should you, as a referring physician, wish for a specific statement or a telephone consultation, please feel free to note this on the referral form. In addition, it makes our work easier if patients also bring current medication plans and, if available, current lab values and, where applicable, CDs/DVDs with imaging already performed.
