We would be very grateful if you would provide your patient with all relevant reports, as well as a referral or admission form stating the corresponding question. Should you, as a referring physician, wish for a specific statement or a telephone consultation, please feel free to note this on the referral form. In addition, it makes our work easier if patients also bring current medication plans and, if available, current lab values and, where applicable, CDs/DVDs with imaging already performed. You are welcome to contact us in advance should you have any questions regarding the procedure; we look forward to hearing from you!
