Referring Physician Information

Dear colleagues, we are happy to care for your patients together with you, and, where you wish, to discuss the further course of action with you.

You can find the flyer for referring physicians here as a PDF.

We would be very grateful if you would provide your patient with all relevant reports, as well as a referral or admission form stating the corresponding question. Should you, as a referring physician, wish for a specific statement or a telephone consultation, please feel free to note this on the referral form. In addition, it makes our work easier if patients also bring current medication plans and, if available, current lab values and, where applicable, CDs/DVDs with imaging already performed.

In our consultation for complex endoscopic questions, we plan and discuss an individual approach. Referring physicians may likewise present their patients for a second opinion. Patients may also present for a second opinion in our specialist consultation, after sending their prior findings to ambulanz-med3@ukaachen.de. More information can be found  here.
